You Qualify:

If You are Certified by a Physician
- Diagnosed with disabling medical
  condition
- Patient must have restorative
  potential

If You Have Medical Necessity
Intermittent skilled nursing care or:
- Physical Therapy
- Speech Therapy
- Occupational Therapy

If You Have Insurance Eligibility
- Home Health Agency performs the
  inquiry

If You Are Homebound
- Require a Licensed Caregiver
- Need assistance with daily activities
- Require daily technological assistance
- Leave home with the assistance to
  attend
- Does not regularly work in
  paid position outside the home
- Leaving home is a major effort

Medicare and Private Insurance companies will help cover home health costs for beneficiaries who meet these four conditions.
